Aging and Disability Summit Scholarship Application

The Colorado Departments of Health Care Policy & Financing, Human Services, and Public Health & Environment, in conjunction with other state, local, and non-profit organizations, are pleased to announce their Call for Sessions for Colorado’s Inaugural Aging & Disability Services Summit: Together, Forging a Path to the Top. The event will be held November 13 – 15, 2012 at the Crowne Plaza Hotel in Colorado Springs, Colorado.

The purpose of this inaugural assembly is to offer high caliber training and professional development for both private and public sector health and human services professionals who provide services to older adults, vulnerable adults, and persons with disabilities. The overall objective of the event is to promote collaboration, innovation, education, skills, and leadership to enhance service delivery to these populations throughout Colorado.

As a result of attending this summit, participants will:

  • Establish and strengthen collaborations across state and local agencies, between oversight agencies and local provider networks, and across systems for all stakeholders;
  • Expand education and awareness of current trends and opportunities; and,
  • Participate in information sharing about evidence-based research, as well as effective practices to support the right person, with the right service, at the right time.

The target audience will include: academicians, advocates, county and state agency staff, caregivers, consumers, family members, funders, health care professionals, self-advocates, service providers, law enforcement, legal community, professionals, researchers, and other interested parties in the field.

The Summit Steering Committee is extending an invitation to policy-makers, service providers, case managers, Area Agencies on Aging staff, county adult financial and adult protection staff, advocates and thought leaders in the delivery of long-term services and supports to submit proposals for concurrent sessions to be included in the program agenda. We invite you to help build a content rich program by submitting concurrent session proposals.The Summit Steering Committee of Colorado’s Inaugural Aging and Disability Summit is committed to making the Summit accessible to people from resource-limited settings and communities. We aim to serve those who, because of financial difficulty, wouldn't otherwise be able to attend.
